Patient Information - Submit Pre Appointment

Intake form before appointment
  • Please fill out this form at least 1 day before your appointment. We are currently open, but take all precautions we can with the COVID pandemic. Please follow our protocols for the safety of our clients and staff:
- Once you arrive at the clinic, we will ask you to call us from your vehicle.
- Please DO NOT enter the clinic. We will then come outside and get your pet, and bring them inside. Please remain in your car. - After your pet has returned to the car, we will ask you to pay by Visa, mastercard or e-transfer to reception@riverroadanimalhospital.ca. We will e-mail you an invoice and you will be ready to go!

  • For dogs core vaccination is Rabies, Distemper, Adenovirus, Parainfluenza and Parvovirus For cats core vaccination is Rabies, Rhinotracheitis, Calicivirus and Panleukopenia.
  • To help prevent Kennel cough - recommended for boarding, grooming, day care or socializing with other dogs
  • Lyme disease is transmitted by ticks and has been found in our area. It can cause serious immune disease in dogs and people. Leptospira can be carried by skunks and raccoons and standing water can be a reservoir. Leptospira can be contagious to people and cause serious liver or kidney disease.
  • Feline Leukemia can be transmitted by bites or close contact with infected cats. It is recommend that cats that go outdoors are vaccinated for Leukemia. Leukemia vaccine can be more reactive and not recommended for cats not at risk.
  • Wellness testing is recommend to have a health baseline, for senior pets or for new pets. For cats that may have had contact with strays or unknown history we recommend testing for Feline Leukemia and Feline Immunodeficiency Virus.
